Help for Teens and Young Adults

Teens experience dramatic changes as they become adults. For those with emotional, behavioral, or mental health challenges, the transition to adulthood can be overwhelming. They may not have all the skills they need, and they are especially vulnerable to unemployment, homelessness, poor educational outcomes, increased substance use, and involvement with the criminal justice system.

The Transition to Independence Process (TIP) program at Southwest Network helps teens and young adults ages 16½ to 22 overcome these challenges and become successful adults.

Following a nationally recognized model, Transition Facilitators provide one-on-one coaching and support to help young people figure out how to find a job or an apartment, go back to school, manage finances, get along with family members, be a better friend, deal with legal issues, or whatever will help improve their quality of life. If needed, we can also assist in the transition from the child and adolescent behavioral/mental health system to the adult system.

Southwest Network assists teen and young adult members with support for:

  • Social and family relationships

  • Independent and daily living

  • Finances and budgeting

  • Decision making

  • Emotional and physical health and wellbeing

  • Community life
