The Child and Family Team Process
Your case management team at Southwest Network will strive to provide the very best services, while helping strengthen your family. We accomplish this by assembling a Child and Family Team, or CFT, which will develop a plan for your unique issues and goals.
The CFT will bring together the wisdom and support of family and friends as well as the resources, experience, and expertise of formal supports such as your case manager, a family support partner, and other professionals. Together, the team will help your family and give you the support you need to go forward.
They will meet as often as necessary, but at least once a month. They will also meet wherever you choose — your home or office, our office, the school, park, library, or any place that’s convenient for you.
Your CFT will start the Child and Family Team process by letting you share your story because you know your family’s situation best. This will help them get to know you and learn about your family’s concerns, needs, hopes, dreams, and strengths. They’ll also want to know about your family’s culture and traditions to understand your unique way of life.
Your CFT will work with you to create goals that will help you get to where you want to be while making sure your child’s physical, educational, and mental health needs are met. These goals will be written in a plan that spells out what services are recommended, as well as simple steps that can be taken to minimize the impact of a crisis.
Throughout the process, we will coordinate the right services to provide care that’s individualized to your child and will help rebuild and strengthen your family. We’ll also help you build a network of people and community resources that will eventually replace us as your support system.
